FANGZ Limited Dental Reimbursement Warranty

Effective Date: June 7, 2026

FANGZ LLC ("FANGZ") stands behind its products and is pleased to offer this Limited Dental Reimbursement Warranty ("Warranty") to eligible purchasers of qualifying FANGZ mouthguards.

This Warranty is a voluntary customer service program funded solely by FANGZ. It is not insurance, is not a contract of insurance, and is not underwritten by any insurance company.

Warranty Benefit

Subject to all terms, conditions, limitations, exclusions, documentation requirements, and verification procedures contained herein, FANGZ will reimburse eligible out-of-pocket dental treatment expenses up to a maximum of Five Hundred Dollars ($500.00 USD) per covered incident.

Eligibility Requirements

To qualify for reimbursement, ALL of the following conditions must be satisfied:

• The claimant must be either (a) the original purchaser of the FANGZ mouthguard or (b) the athlete registered as the authorized user during the warranty registration process.
• The mouthguard must have been purchased through an official FANGZ sales channel or an authorized retailer.
• The mouthguard must be registered with FANGZ within thirty (30) days of purchase.
• Registration must be completed through the FANGZ Warranty Registration Portal and requires submission of all requested registration information, including proof of purchase, product information, and fit-verification photographs.
• Registration must include: purchaser name and contact information; athlete/user name; proof of purchase; product identification information; clear photographs demonstrating successful custom fitting; and confirmation that FANGZ fitting instructions were followed.
• Failure to complete registration within thirty (30) days of purchase renders the product ineligible for reimbursement under this Warranty.
• The mouthguard must have been properly fitted according to official FANGZ boil-and-bite instructions before the incident occurred.
• The mouthguard must have been worn properly and continuously at the time of injury.
• The injury must occur during an organized league, school, club, camp, tournament, or sanctioned athletic competition supervised by an officially designated referee, umpire, official, coach, or league representative.
• The incident must be reported to FANGZ within fifteen (15) days of occurrence.
• All claim materials must be submitted within sixty (60) days of treatment.
• The claimant must fully cooperate with any claim investigation conducted by FANGZ.

Registration Requirements

Registration is required to participate in the FANGZ Limited Dental Reimbursement Warranty Program. Failure to complete all registration requirements within thirty (30) days of purchase will render the product ineligible for reimbursement. FANGZ's registration records shall control in the event of any dispute regarding registration status.

Required Claim Documentation

A complete claim submission must include:

• Completed FANGZ claim form
• Original proof of purchase
• Confirmation of completed warranty registration
• Registration photographs showing the properly fitted mouthguard
• Documentation demonstrating the claimant is the registered user
• Photographs of the mouthguard following the incident
• Dental records describing the injury
• Itemized dental invoices
• Explanation of Benefits (EOB) from all applicable insurance carriers
• Proof of payment for unreimbursed expenses
• Incident report from the athletic event, if available
• Any additional information reasonably requested by FANGZ

Reimbursement Limitations

• Reimbursement is limited solely to actual documented out-of-pocket dental treatment expenses.
• Reimbursement is available only after all applicable insurance benefits and other available reimbursement sources have been exhausted.
• The maximum reimbursement is $500.00 USD per incident.
• Only one approved claim may be paid per registered user during any rolling twelve (12) month period.
• Reimbursement shall never exceed the claimant's actual documented financial loss.

Exclusions

This Warranty does not cover:

• Orthodontic treatment or appliances, braces, retainers, aligners, or expanders
• Cosmetic dental procedures, veneers, bonding, or whitening
• Future treatment plans or projected expenses
• Pain and suffering, emotional distress, lost wages, or travel expenses
• Soft tissue, lip, tongue, jaw, facial, or TMJ-related injuries
• Concussions or head injuries
• Pre-existing dental conditions or previously repaired teeth
• Dental implants, crowns, bridges, dentures, or prosthetic devices
• Damage from intentional acts, fighting, or non-sport activities
• Recreational, pickup, unsanctioned, or informal play
• Injuries occurring when the mouthguard was not worn, not properly fitted, or was altered or misused
• Products purchased from unauthorized sellers or counterfeit products
• Products not registered within thirty (30) days of purchase
• Injuries occurring more than one (1) year after the original purchase date
• Claims involving fraudulent, misleading, or inaccurate information

Right to Verify

FANGZ reserves the right to review, investigate, verify, approve, deny, or request additional information relating to any claim. Failure to cooperate may result in denial.

No Guarantee of Payment

Submission of a claim does not guarantee reimbursement. Failure to satisfy any eligibility requirement, documentation requirement, limitation, exclusion, or verification request may result in denial.

Limitation of Liability

Except for reimbursement expressly approved under this Warranty, FANGZ shall have no liability for any direct, indirect, incidental, consequential, special, exemplary, punitive, or other damages arising from the purchase, use, misuse, or inability to use any FANGZ product.

Warranty Changes

FANGZ reserves the right to modify, suspend, discontinue, or replace this Warranty at any time for future purchases. Claims relating to products purchased prior to any modification shall be governed by the Warranty terms in effect on the original purchase date.

Photo and Content Submissions

Photographs and other materials submitted as part of registration or claim administration may be retained and used by FANGZ for warranty administration, claim review, fraud prevention, product safety evaluation, legal compliance, and internal business operations. Any separate authorization allowing FANGZ to use submitted content for marketing or commercial purposes shall be voluntary and is not a condition of eligibility under this Warranty.
